JOB SUMMARY: Carries out the Activities program designed to meet the individual needs of the residents in the facility. Keeps ongoing and accurate progress notes and attendance records of all residents to and from activities, and gives input into monthly activity calendars for the units and adult day services program. Consults with the Director on special needs and wants of residents. Together with the director ensures a seven-day program. QUALIFICATIONS: Education: High School Diploma or GED Experience: One-year experience in providing activities or experience in a health care setting preferred. Documentation experience. Job Knowledge: Documentation skills. Other: Outgoing personality, special talent that can be applied toward activity program is preferred. Must be able to read, write, and speak the English language. Responsible for actively demonstrating Putnam Ridge’s “PRIDE” philosophy of professionalism, responsibility, integrity, dedicated service and excellence. ESSENTIAL FUNCTIONS: • Assists in providing recreational and other activities as per the individualized activity interests and multidisciplinary care plan. • Leads or facilitates various activity groups. (ie: religious services, physical and cognitive games, arts and crafts, gardening, grooming etc. • Communicates with residents with visual, speech, auditory, cognitive, physical and emotional impairments. • Regularly assess the social and emotional needs of the residents with the Director to assist Director in setting appropriate goals for meeting those needs within the overall plan of care. • Helps in keeping activity materials organized. • Helps decorate the facility with seasonal decorations. • Gives input on monthly activity calendars, and hangs them on appropriate units. • Reports all cases of alleged violations involving mistreatment, neglect or abuse, including injuries of unknown sources. • Answers call bells in a timely and polite manner. • Other duties and responsibilities as assigned. PHYSICAL REQUIREMENTS: • Transporting residents to and from in-house activities and accompanying residents to and from community programs. • Position requires movement throughout the entire building. • Pushes 225 lbs. In a wheelchair, recliner, or Geri-chair. • Leading appropriate exercise classes (pushing, bending, stretching) EXPECTATIONS: Treat residents/patients with respect, dignity and compassion. Is confidential. Is cooperative and considerate to fellow employees. Is available and willing to assist as needed. Is neat, clean, and properly attired as per dress code.

Minimum Salary: 17.75 Maximum Salary: 17.75 Salary Unit: Hourly
