Picture yourself here: When you join PPGNY as a Lead Navigator you will act as point of contact for for grant clients, while also providing the following services: education, appointment scheduling support, follow-ups, case management, benefits navigation and HIV testing.

Additional responsibilities include:

  • Intake and ongoing assessment of health.

  • Outreach support in our Bronx Health Center and on the Mobile Medical Health Center (MHC) shifts. Participate in engagement and education of priority populations regarding HIV Counseling and Testing, PrEP, and access to health care and supportive services.

  • Provide linkage to care for persons with HIV (PWH) work are newly diagnosed or out of care while following PPGNY HIV Testing policies and procedures.

  • Educate and enroll consumers into PrEP as per PPGNY policies and procedures.

  • Appointment support and conduct follow-up activities.

  • Overdose Prevention training in the office and in nontraditional settings as per Project Street Beat policies and procedures.

  • Represent program at internal and community meetings, workshops, conferences

  • Promotes PSB in the community by developing formal and informal linkages by making presentations to service providers and other agencies

  • Enter complete required data into required data systems within 48 hours of completing a service, current PSB data systems include but are not limited to eClinical Works (ECW), Efforts to Outcomes (ETO) and eShare.

  • Work collaboratively with the HIV Prevention Specialists and Case Managers to engage, enroll and facilitate services: HIV Counseling and Testing, PrEP, Seeking Safety, and Overdose Prevention training.

You have:

  • Familiarity with social services, specifically HIV prevention and care.

  • High school diploma/GED

  • Two (2) years of experience providing direct support to vulnerable, populations experiencing homelessness, substance use, mental health issues, and/or living with HIV/AIDS

  • Two (2) years of experience providing HIV counseling and testing in an office and or nontraditional setting.

  • Two (2) years of experience facilitating support groups and or providing trainings.

  • Ability to work a flexible or extended schedule, including evenings and weekends, as operational demands require

All positions at PPGNY require:

Proof of immunization or immunity to certain communicable diseases (including influenza during the flu season) and testing for tuberculosis. These certifications are required by the NYC DOHMH Health Code, NYSDOH and OSHA.
