Reports To: Program Coordinator, Single Stop Program
Location: 503 5th Avenue, Brooklyn, New York 11215
Hours: 20 Hours/Week (Mon – Fri, 10AM – 6PM)
Job Summary: The Single Stop intern will assist participants with a wide array of social services including: screening for appropriate benefits, application and re-certification assistance for varied services and benefits, accessing affordable housing, advocacy for benefits and services, and community outreach to inform about the program’s services.
Conduct respectful, accurate, and timely intake, assessment, and service planning with all assigned program participants, entering information into required databases as indicated
Work in an ongoing manner with participants to provide direct services and referrals to address their immediate needs as well as longer-term concerns in order to help them achieve their goals and enhance their self-sufficiency.
Provide advocacy, outreach, and make referrals to government agencies, social service providers, elected officials, and other programs on behalf of participants
Collaborate closely with other Single Stop Case Managers to ensure community residents benefit from Single Stop services and benefits.
Direct services: provide one-on-one assistance with the following services:
Safe Link and Assurance cell phone new applications and renewal/replacement
MTA Reduced Fare MetroCard application for seniors and disabled
Complete SCRIE and DRIE applications
Help with housing applications and recertification (.e., NYCHA, NYC HOUSING CONNECT, Senior Housing, and other low-income applications, etc.) via the online NYCHA portal or internet)
Birth certificate replacement and new applications
Social security replacement card
EBT card replacement
Connect seniors to various housing options in NYC
Medicare and Medicaid new application and replacement card
SNAP application and recertification via HRA ACCESS NYC
Pre-screen participants for free tax preparation during the tax season
Perform other duties as assigned
Bilingual -- English/Spanish required
Must have good written and verbal communication skills
Must be willing to work using a strength-based, youth and family development approach
