What will my day look like?

  • Assesses patient needs, and develop nursing care plan for care coordination and linkage with community resources

  • Completes medical assessments for behavioral health and substance use disorder clients, provides integration/health education, and develops nursing care plan to provide holistic and individualized treatment to address each patient’s needs.

  • Assists patients to achieve their treatment goals through health education. Will provide one on one education to residents and teach Medical Aspects Groups.

  • Provides therapeutic environment and fosters team approach with multidisciplinary staff.

  • Evaluates patient responses to program. Reports to physicians, nurse practitioners, and counselors both objective and subjective data of patient behaviors.

  • Utilizes nursing diagnosis; identifies priorities of care; sets realistic short-term goals and outcomes, monitors progress towards goals and adjusts care coordination accordingly.

  • Implements nursing actions that promote, maintain, and encourage healthy lifestyle behaviors.

  • Works collaboratively with the team and identifies goals, lifestyle and behavioral modifications, and appropriate nursing interventions for each patient.

  • Identifies health education needs and knowledge deficits of the patient. Provides appropriate health teaching through multimedia materials; and documents health teaching and responses in the group notes.

  • Able to identify patient needs for community services and provide linkage.

  • Develops professional interpersonal relationships with the members of the multi-disciplinary team, residents, and families.

  • Attends and participates in all staff meetings and mandatory in-services. Provides appropriate ideas and suggestions to improve process or operations of the facility.

  • Must demonstrate proficiency and accuracy in the administration of all ordered medications, and knowledge of appropriate use of medication and adverse effects.

  • Will assist Medical/Psychiatric providers with integration, medication management, and psychiatric visits.

  • Will demonstrate proficiency with computer skills and provide accurate documentation for medical records/EMR.

  • Able to follow physician/provider orders and recommendations for medical treatment of patients.

  • Provides a safe environment for patients, staff, and families and considers safety in all interactions.

  • Identify and triage medical emergencies and secure appropriate medical treatment for patients, family, or staff.

  • Will attend educational conferences, webinars, and seek out opportunities for professional growth and development including attainment of advanced degrees and certifications.

  • Must demonstrate proper procedure and technique during venipuncture, collection, and processing of blood samples; and collection and testing of urine toxicology and HCG samples.

  • Must be comfortable providing telehealth services

What has prepared me for this opportunity?

  • 2- 5 years’ clinical experience

  • Experience working with mental health and substance use disorder patients preferred.

Formal Education/Training: Graduation from a regionally accredited or New York State registered college or university with a Nursing Degree. Associates Degree preferred. Candidates must have current New York State professional nursing license to practice as a Registered Nurse.

Licensures/Certificates: NYS – RN, Certificate of CPR/AED Training and First Aid.
