Advance Care Alliance is an exciting Start-Up Care Coordination Organization providing community-based services and support to people with Intellectual and Developmental Disabilities throughout NYC, Long Island & the Lower Hudson Valley.

  • Ensure that the person centered needs of individuals receiving services are met.

  • Facilitate Life Plan meetings; develop, write, monitor and implement the Life Plan.

  • Provide advocacy for people receiving services to assist them to reach their full potential.

  • Ensure benefits are maintained (Medicaid).

  • Record and maintain documentation in the Care Management Platform.

  • Travel throughout the designated service area

  • A Bachelor of Arts or Science degree with two years of relevant experience or a Master’s degree with one year of relevant experience.

  • Absolute sense of integrity and personal commitment to serving people with I/DD and their families.

  • Excellent interpersonal, public speaking, and written communication skills.

  • Ability to work autonomously.

  • Bilingual is a plus!

ACANY is a start-up 501(c)(3) organization that has received designation as a Care Coordination Organization/Health Home (CCO) from New York State. CCO is an emerging care management model for people with I/DD that combines Medicaid service coordination (MSC) of home- and community-based services (HCBS) with integrated care management of a person’s medical and behavioral health needs. The organization’s primary mission is to provide a smooth transition to this enhanced service model for people with I/DD and their families. ACANY supports 25,000+ people in its program spanning New York City, Long Island, and the Lower Hudson Valley.
