Bassett Healthcare Registered Nurse Nursing Care Coordinator in Herkimer, New York

Responsibilities The Registered Nurse Nursing Care Coordinator will work Regular Full-Time Days, Flexible 8 or 12-hour shifts, works limited weekends and holidays: one weekend a month and one winter and one summer holiday. The Registered Nurse Nursing Care Coordinator will work on our160 bed skilled nursing facility. The Registered Nurse Nursing Care Coordinator will lead a care team to develop and implement individualized plans of care for nursing home and subacute care residents. The Registered Nurse Nursing Care Coordinator will provide nursing assessments, treatments and working directly with the medical team on medical orders. The Registered Nurse Nursing Care Coordinator will have daily interactions with residents and their families. The Registered Nurse Nursing Care Coordinator will be responsible for directing and supervising nursing personnel working the nursing unit. Education/Qualifications: Graduate of an accredited school of nursing and currently licensed by NYS as a Registered Professional Nurse. One (1) year as general staff nurse and two (2) years of nursing supervision preferred.
