Elderwood/Pediatric/PostAcute/Woodmark Activities Assistant in Hornell, New York

Elderwood at Hornell is seeking an Activities Assistant to join our team and be an important part of enriching the lives of residents.

Responsible for providing recreational activities and services that meet the individual needs and interests of our residents. Creates a pleasant, social, and caring atmosphere for residents at all times. Utilizes recreation or creative art activities to maintain and achieve physical, cognitive, social, emotional, and/or spiritual health. Other responsibilities include: completing Activity Initial Assessments, completing MDS, creating care plans, documenting progress notes as needed, delivering resident mail, helping to clean fish tanks, maintain activity participation documentation, delegate tasks to volunteers/students, assist on out trips in the community, arrange furniture to accommodate activity programs, transport residents to and from activity programs in wheelchairs, and light housekeeping of activity areas.

  • Assists with and/or independently leads activity programs (including Seasons Program), as assigned. Such activities may include but are not limited to exercise programs, art/craft groups, reminiscent programs, sensory, bingo, social events, outings, church services, or discussion groups.

  • Assists in the planning of activity programs (including Seasons Program) and in their implementation, including outings.

  • Completes Initial Activity Assessments within 72 hours of residents admission, as assigned

  • Develops resident care plan based on assessed leisure needs and interests prior to initial care plan conference- Ensuring that goals are individualized to match skills, abilities and interests.

  • Assists with delivering and posting monthly activity calendar in resident’s rooms, in common areas, and on large marker boards. Completes Minimum Data Sets per schedule.

  • Attends/participates in care plan conferences as assigned or in the absence of the Activities Director.

  • Assists residents to and from activities and signs residents out when taken to Off-Unit Activities. Is responsible for reminding co-worker CNAs to assist their assigned residents, who wish to attend group activities, to the activity room and announcing group activity programs over the intercom ½ hour prior to start of program to alert residents of the event (as seen appropriate).

  • Assists with resident out-trips –Signing residents up for shopping/outings, assisting them with their shopping lists/signing out funds from account per their request, helping them to get on/off bus and escorting them during outings.

  • Organizes and sets up supplies needed to activity programs in an adequate amount of time prior to scheduled programs.

  • Helps to provide adequate space and attention to groups and individuals in groups.

Maintains daily activity participation attendance records and other required documentation such as fluid intake as required (notified nursing staff of resident’s intake)

Leads Resident’s Council Meeting in the absence of SW & Activities Director, as assigned

  • Interacts with residents, their families and visitors and with supervisors and co-workers in a polite and professional manner.

  • Provides individual activity and friendly visits as needed or per care plan, with compassion, understanding and patience. – Helps to write/mail letters, reads to residents, offers sensory stimulation, turns on music, massages hands, etc.

  • Supports and maintains a culture of safety and quality.

  • Notifies appropriate licensed staff when resident complains of pain.

  • Assists residents with behavioral problems, when in groups, to an area within the room that is more conducive to appropriate behavior or assists residents back to their rooms per their request.

  • Treats residents and their families with respect and dignity. Approaches all residents in an open, friendly, and gentle manner supported by appropriate body language. Handles situations with tact.

  • Must ensure that an appropriate person is left in attendance, if required to leave the group at any time.

  • Carries out scheduled activities of the day (includes delivery of mail/newspapers), unless extenuating circumstances prevent doing so. Must notify the Activities Coordinator when this occurs.

  • Provides resident with encouragement, praises accomplishment(s). Verbally reminding them of daily and upcoming activity programs of potential interest

  • Utilizes volunteers in assisting with activities: Assignments may include watering flowers/plants, delivering/reading mail and newspapers, organizing residents belongings, taking resident for a walk, transporting residents to/from activity programs, outings, or visiting 1:1

  • Prepares and assists with feeding residents, as needed during activity programs. Provides sufficient drinking water, fluids, juices and snacks during activity programs and outings, documents fluid intake.

  • Has a genuine interest in and appreciation of adults in the later years of life.

  • Assists with cleaning and maintenance of fish tanks and feeding of the fish`

  • Notifies Activities Director or Nursing Supervisor on weekends if unable to report to work. Reports to RN on duty when Activities Director or Nursing Supervisor are not available.

  • Explains all aspects of care to the resident before performing hands on care

  • Notifies Activities Director when supplies are low or equipment is not functioning properly – Assists with running errands for supplies and materials

  • High school graduate

  • NYS Nurse Aide Certification highly preferred

  • Adheres to dress code, appearance is neat and clean.

  • Completes annual education requirements related to Corporate Compliance Education Plan.

  • Maintains regulatory requirements.

  • Maintains resident confidentiality at all times.

  • Reports to work on time and as scheduled, completes work within designated time.

  • Wears identification while on duty, uses computerized punch time system correctly.

  • Completes in-services and returns in a timely fashion.

  • Attends staff meetings as scheduled.

  • Represents the organization in a positive and professional manner.

  • Actively participates in performance improvement and continuous quality improvement (CQI) activities.

  • Complies with all organizational policies regarding ethical business practices.

  • Communicates the mission, ethics and goals of the facility, as well as the focus statement of the department.

  • Maintains resident confidentiality and respects residents rights per facility policy
