Lutheran Community Life Coordinator- Part Time in Jamestown, New York

Position Title: Community Life Department, Community Life Coordinator (part time)

Facility & Division Name: Lutheran Home and Rehabilitation Center

Purpose of this position:

The purpose of this position is to provide activities that meet past and current leisure interests of each individual resident. This position will be directly responsible for providing activities that benefit each resident while meeting both psychosocial and physical needs. This position will be a member of the Interdisciplinary Team and will be directly responsible for CAA review and care plan development. The person must be able to self- manage their time and possess strong organizational and communication skills.

Qualifications

A high school diploma required.

Associates Degree preferred: Certified Occupational Therapy Assistant or related field.

Minimum of 1-year Long Term Care experience. Other relevant geriatric experience considered.

Basic working knowledge of computers and Microsoft Office.

General Hours:

9am-5pm Saturday and Sunday

Varied Evenings 4pm-8pm

Job Description:

  • Upon admission, complete an assessment and begin development of anticipated involvement in activities programs.

  • Develops activities plan that meets with the attending physician’s approval and completes ongoing review and changes to meet the resident’s needs.

  • Reviews CAA’s generated by the MDS and develops care plan using this information.

  • At regularly scheduled meetings, incorporates the activities assessments, approaches and goals with the care plan team for each resident.

  • Writes detailed progress notes to indicate individual response and involvement at least quarterly and more often as needed for each resident.

  • Completes discharge summaries according to facility policy for residents.

  • Maintains accurate records of attendance for each activity covered.

  • Completes a monthly attendance activity report to be given to the activity’s director.

  • Plans and carries out a schedule of programs to meet the individual activity plans for each resident. Evaluates and changes as necessary.

  • Coordinates activity programs with all departments as needed.

  • Posts, announces and informs resident, staff and families of the scheduled activities programs.

  • Provides a calendar to resident and families as requested and appropriate staff.

  • Maintains records of purchases made for the unit.

  • Provides a monthly budget report to Activities Director on a regular basis.

  • Decorates the floor for current season or event.

  • Maintains an inventory of supplies.

  • Plans and carries out an organized and meaningful placement for students from area colleges as assigned.

  • Evaluates students with Activities Director and college representatives.

  • Makes volunteer needs known to the Volunteer Coordinator.

  • Orients volunteers to specific tasks, evaluates volunteer effectiveness and assists with recognition events.

  • Attends in-services, courses and seminars that would enhance the activities program.

  • Participates in professional organizations as desired.

  • Assists residents to fill out applications for absentee ballots.

  • Is flexible and adjusts scheduled hours to meet resident needs. May work beyond regularly scheduled hours or may need to cover absences of peers.

  • Coordinates room and equipment use.

  • Completes dietary slips for weekly stock orders and special events.

  • Provides direct, regular contact with each resident to help in determining resident’s mental and emotional status and unmet needs and to determine the appropriate resolution of these needs.

  • Supervises pets and informs Activities Director of any necessary veterinary care.

  • Manages multiple priorities with a minimum of supervision.

  • Organizes programs and large activities effectively.

  • Assists staff and residents in dealing with grief.

  • Communicates well in oral and written form.

  • Participates in public speaking on behalf of the organization as needed.

  • Works well with other departments and works to eliminate conflict.

  • Seeks out new methods, uses creativity and new principles to stimulate residents and shares this insight with peers.

  • Operates telephone, fax, VCR, stereo, slide projector, video camera, still camera, microwave, oven, computer, calculator, etc.

  • Collaborates with Nursing regarding appropriate activities to meet the resident’s needs and illustrates this in the care plan.

  • Attends Morning Report daily and offers input on resident changes.

  • Adheres to the Lutheran QAPI (Quality Assurance Performance Improvement) Plan by participating in efforts to improve the way we care for and engage with our residents and contributes to the solution for improving outcomes.

  • Safety – Follows required safety procedures.

  • In-services – Attends all required in-service training.

  • Other duties as assigned.
