When you join PPGNY as the APC, Centralized Follow-Up (CFU) you will spend most of your time on the computer in a virtual environment providing care to diagnose and treat issues and conditions related to reproductive, sexual health, abortion, and gender-affirming care. This person will also spend their time using the Electronic Health Record system EPIC, reviewing labs and diagnostic imaging, triaging patient’s questions, handling prescription requests, documenting patient interactions, and running reports. Some of your time will be spent interacting with CFU team members, other APCs, collaborating with other departments and consulting with PPGNY program directors. This position will require weekend work on a rotating schedule. The person in this role will know they are successful when all patients who receive follow-up care have their needs met in a patient-centered manner and in accordance with PPGNY’s Medical Standard and Guidelines (MS&Gs) and policies. The person in this role may work from home or the office but may be required to be in the office occasionally for administrative duties, training, or team building.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
Patient Care- (95%)
Daily review of centralized follow up pools
Manages abnormal labs and creates management plan as per PPGNY’s MS&Gs
Writes prescriptions for patient treatment, birth control and other medications as indicated.
Manages abnormal referral reports and creates management plan in accordance with PPGNY MS&Gs
Consults with physicians as required to develop treatment plans or for general management advice
Performs chart review for patients in follow-up to determine recommendations when services not obtained according to PPGNY MS&Gs
Initiates referrals as needed
Reviews all normal and negative labs
Reviews normal outside referral reports from diagnostic imaging and specialists
Responds to refill requests: sending in RX or denials per PPGNYs MS&Gs
Triages, counsels, educates, and manages patient questions and concerns via phone or patient portal according to PPGNY’s MS&Gs, policies and protocols
Manages urgent labs for example Beta HCG, Rh, confirmatory HIV, and PrEP, and PEP labs
Generate reports
Documents accurately, appropriately and efficiently all phone interactions with patients in the EMR, EPIC
Expedites patient scheduling for time sensitive services such as STI treatment, colposcopy, etc.
Demonstrates ongoing proficiency in EPIC especially using In Basket
Responds to prior authorizations for medications and services as needed.
Supports licensed and unlicensed staff with medical questions or concerns
Administration- (5%)
Participates in onboarding and training of new centralized follow-up staff.
Provides excellent customer service to both internal and external customers by working towards a positive outcome with any problems encountered
Works closely with the CFU director to identify and prioritize EPIC configuration needs related to notification and follow up.
Performs other duties as assigned

REQUIRED QUALIFICATIONS
Licensures/Certifications - New York State license as a Physician Assistant, Midwife or Nurse Practitioner and be certified by a national credentialing body.
New York registration as a Nurse Practitioner, Physician Assistant, Nurse Midwife or Certified Midwife.
Computer literacy
Minimum 1-year of clinical experience as an Advanced Practice Clinician providing sexual and reproductive health services
PREFERRED QUALIFICATIONS
ACLS/BLS Certified
Bilingual in English and Spanish.
