Overview
NADAP is hosting an in-person job fair on Wednesday, July 10th

Salary: $45,000-$50,000 depending on education level, plus more for bilingual English/Spanish; productivity bonus incentives
Work Schedule: Hybrid flexible four or five day work week based in our Midtown Manhattan location.

Responsibilities: Care Managers help individuals achieve improved healthcare outcomes. Our teams provide field-based support to assist individuals with medical and behavioral health conditions
through the following services:
• Assessment
• Care Planning
• Connections to Primary and Specialty Care Providers
• Service Coordination with Treatment Providers
• Client Education on Diagnoses, Medications and Treatments
• Linkages to Community-based Resources/Entitlements

Qualifications
Requirements:
• Bachelor’s Degree in human services, social work, or related field; or Associate’s degree with 1 year of case management experience; or High School Diploma with 2 years of case management experience.
