Organization Overview:The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We are two of New York City’s leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults through-out Manhattan and New York City. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational.
Intro/Program Description:LSNC is a multigenerational program of Goddard Riverside that serve educational, recreational, cultural needs and aspirations of Lincoln Square community with a focus on residents of Amsterdam Houses and Amsterdam Addition. LSNC Aging Services are funded by Department for the Aging and The New York State Office for the Aging (SOFA). More than 600 active members are offered daily nutritious hot lunch as well as a variety of wellness, educational, art programs and social services. Senior services provide an opportunity for older adults (age 60+) to lead more vibrant and meaningful lives through comprehensive programming and social support including case management, case assistance and health promotion. The NORC serves residents age 60+ who are residents of the NYC Housing Authority developments Amsterdam Houses and Amsterdam addition with social services, health promotion and recreational/cultural/social opportunities. About 50% of the seniors are Spanish-speaking. The NORC is run in conjunction with a senior center that is open to all New Yorkers ages 60+.
Purpose of Position:Under supervision of LSNC Aging Services Program Director and Social Services Supervisor, Case Assistant provides services that includes new clients registrations and annual assessments, updating client information and re-assessing client needs on annual basic for assistance with applications and recertification for benefits, referrals and connections to services such as post-discharge homecare, legal services and medical care; advocacy with public and provide systems; collaboration with the on-site nurse; telephone reassurance and friendly visiting, home visits to ensure clients are safe and well.
Schedule:Monday through Friday, 8:00 am to 4:00 pm. Weekends: Cooling Center Administration if required (occasionally, summer only, rotation schedule)
Salary Range:$31,164 - $38,955
Roles, Responsibilities, and Essential Duties
- Client services are provided under Director and Social Services Supervisor:
Provide supporting advocacy, referral and information connecting to entitlement/benefits counseling, health and mental health care, food assistance and housing, caregiver referrals as well as other program components as required by program guidelines
Perform annual assessments and make home visits to homebound seniors to identify their needs, link them to services and provide ongoing social contact
Document services provided and maintain accurate client records, written and electronic and enter into client database(s) in a timely manner, according to program and agency guidelines
Work with onsite nurse to coordinate medical care and provide medication management
Ongoing appointment coordination and reminders
Emergency assistance and outreach if needed
Provides Friendly Visiting and Telephone Reassurance calls
Assist with signing up new members, client registration and complete annual assessments, re-registrations, and health indicator program requirements
- Aging Services Team member:
Attend staff meetings, supervision sessions, training programs and case conferences
Participate in program assessment and improvement plans
Assist in daily oversight of senior center and respond to emergencies
May be asked to assist with lunch tickets distribution and front desk if needed
Assist with outreach and engagement
- Remote Duties:
If necessary, provide services remotely
Meet with clients as needed at senior center for document drop off /pick up of documents for review and assistance from homebound clients if they are unable to complete drop off at the center
- Other duties as required.
Qualifications/Educational Requirements:
High school diploma required
Fluency in Spanish is a must
Skills, Knowledge and Abilities:
Sensitivity to the strengths and needs of older adults
Strong work ethic
Ability to multi-task and work in a fast-paced environment
Ability to write concise and clear case notes
Ability to communicate with clients who may have disabilities (physical, hearing impairment, visual) and/or mental illness
Computer Skills:
Able to use Microsoft Office Suite programs
Able to use or learn electronic databases
Able to use DFTA PeerPlace ( training will be provided)
Physical Requirements:
Ability to circulate in lunch room for about an hour a day and to greet clients in hallway to accompany to office
Preferred: ability to walk and stand frequently
Ability to travel to homebound clients and hospitals if needed
Work Environment:
Senior center environment/NYCHA facility
Shared office space/ second floor accessible by elevator
Occasional home visits and fieldwork to accompany participants to appointments or to follow up post-hospitalization
