Advance Care Alliance Of NY Inc. Clinical Outcome Specialist - Clinical Services (OPWDD) in New York, New York

Advance Care Alliance (ACANY) is seeking a Clinical Outcomes Specialist to join a regional clinical services team, comprised of social workers, licensed mental health professionals, and nurses. The clinical services team works closely with the ACANY care management team to:

  • Integrate services and supports

  • Interface with community supports, like hospitals, behavioral health IPAs, and NYS START programs

  • Train the care management team on a variety of health-related topics and provide support

  • Central point of contact for scheduling Coordinated Assessment System (CAS) interviews with Maximus, along established timeframes.

  • Works in conjunction with Care management to ensure social determinants of health and overall health needs are met

  • Reviews data to identify at risk and high clinical need members

  • Ensures a person-centered planning approach

  • MSW, MHC, or MA in relevant Human Services Degree is required.

  • Absolute sense of integrity and personal commitment to serving people with I/DD and their families;

  • Experience providing direct service and intervention to individuals in the community, preferably with some experience in crisis services, substance abuse treatment, or continuity of community health supports;

  • Excellent interpersonal, public speaking, and written communication skills;

  • Ability to work autonomously;

  • High level of professionalism, respect and ability to work in a team environment.

ACANY is a 501(c)(3) organization that has been designated as a Care Coordination Organization/Health Home (CCO) by New York State. CCO is a care management model for people with I/DD that combines Medicaid service coordination (MSC) of home- and community-based services (HCBS) with integrated care management of a person’s medical and behavioral health needs. ACANY supports 25,000+ people in its program and draws Care Managers from 100+ member organizations spanning New York City, Long Island, and the Lower Hudson Valley.
