City of New York Special Consultant Level I Supervisor in New York, New York

Job Description

The Office of School Health (OSH) is a joint program of the New York City Department of Education (DOE) and the New York City Department of Health and Mental Hygiene (DOHMH). Office of School Health provides health services and preventive services to Department of Education students.

The Asthma Case Management Program (ACMP) is a component of the Office of School Health medical services since 2016. Asthma Case Managers are placed in selected community schools in the Bronx to partner with the OSH nurse and school physician as part of the OSH medical team to support students

with a diagnosis of asthma while in school.

The Special Consultant Level I will be trained as a supervisor to the asthma case managers (PHE) who will work closely with families to assure that they understand the principles of asthma management in schools and community. The Special Consultants Level I will report directly to the Special Consultant Level II who will train them and supervisor them on the Asthma Case Management Program (ACMP) protocols and training manual.

Job Duties and Responsibilities:

  • Provide direct supervision to 8-10 case managers in the program and be cross trained for all CMs in the program.

  • Supervise the asthma case managers both at headquarters and at their schools.

  • Coordinate ongoing training and reviews to case managers throughout the school year.

  • Monitor implementation of case management plans and outcomes.

  • Reach out to parents in completing Medication Administration Form for students when there are barriers for the case managers in the program.

  • Outreach to parents in completing the Student Asthma Questionnaire when there are barriers for the case managers in the program.

  • Teach back spacer technique with diagnosis asthma students to the case managers in the program.

  • Train on the Automated Student Health Record (ASHR), and document in the case management section.

  • Oversee, review, and provide feedback to the CMs on their documentation in ASHR.

  • Train the case managers on the guidelines of the ACMP manual.

  • Train the case managers on documenting on the DBN log and reviewing their documentation.

  • Visit case managers at their schools and providing 1:1 supervision on a monthly base.

  • Attend and lead the Monthly Case Reviews for the program at the case’s manager’s school.

  • Meet with the school principals to follow up on issues regarding the CMs work.

  • Maintain weekly case management reports.

  • Maintain case management logs on their supervisees.

  • Complete annual Performance Evaluations and yearly Task & Standards as implemented by the Program Manager.

  • Serve as additional educational resource for students with asthma and their families.

  • Summer detailing project and OSH projects during Spring and Winter Breaks.

  • Participates in agency-wide required activities for emergency preparedness as per the directive of the agency, OSH Medical Director and Program Manager.

Qualifications

A baccalaureate degree from an accredited college or university, and either:

  1. A Master's degree in Social Work from an accredited college or university in the school of social work, or 60 graduate semester credits from an accredited university in social work, psychology, health, rehabilitation, public health, psychiatric nursing or special education; plus one year of full-time, satisfactory experience providing direct care in a social, psychiatric, health, mental retardation/developmental disabilities or substance abuse/chemical dependency/alcoholism agency or in the administration of a

program providing direct care as described above, or as a consultant on program planning or evaluation for such services; or

  1. A Master's degree of at least 30 graduate semester credits from an accredited university in psychology, sociology, anthropology, public health, special education,

psychiatric nursing, counseling, human services, health, rehabilitation, public administration, or business administration; plus two years of experience as described in "1" above; or

  1. A satisfactory combination of education and/or experience. Candidates without either a Master's degree as described in "1" or "2" above or at least 60 graduate semester credits in the areas listed in "1" above, must have at least:

a) a baccalaureate degree from an accredited college or university and one year of actual experience as described in "1" above; plus

b) a satisfactory combination of: (i) graduate semester credits in the areas listed in "1" and "2" above; and/or (ii) additional experience as described in "1" above, to equal 60 graduate semester credits or 2 years of experience. Graduate semester credits from an accredited university may be substituted for the experience in qualification "3 b)" only, on the basis of 30 graduate semester credits for each year of experience. Experience may be substituted for graduate semester credits in qualification "3 b)" only, on the basis of one year of experience for each 30 graduate semester credits.

NOTE: For assignment to Assignment Level II, in addition to meeting the above qualification requirements, all candidates must have one additional year of experience as described in "1" above.
