Bonadio Group, The Financial Counselor/Case Manager in Oswego, New York

We are in need of a a Financial Counselor or Case Manager responsible for discussing and understanding the financial arrangements and obligations of the resident at an elder care services client of ours. As the Financial Counselor you will serve as the financial case manager between the elder care facility, a resident’s responsible party, Medicare/Medicaid, Social Security and other third parties. Duties will include new admission orientation, coordination of Medicaid application processes with DSS and provide other financial counseling as required.

Primary Duties:

  • Perform a financial orientation with resident’s responsible party within 48-72 hours of admission. Duties include:

  • Scheduling of appointments (evening hours as required)

  • Review of completed admission agreement/application and related documents

  • Perform a full financial review to determine ability to pay and identify potential MA applicants

  • Educates families on insurance coverage

  • Discuss current and future financial obligations to Baptist Health

  • Explain resident funds, telephone and additional Baptist Health services available

  • Establish a relationship that will allow for a proactive exchange of information

  • Work with the Director of Admissions on pre-admission screening

  • Coordination of the Medicaid application process. Duties include:

  • Indentifying potential Medicaid applicants in a timely manner

  • Work with the responsible party in completion of the MA application and the gathering of information

  • Act as the conduit between Facility, residents and DSS

  • Establishes direct deposit of NAMI funds (including MA pendings)

  • Monitor census for changes in payor status

  • Completion of income certification forms

  • Completion of VA benefit application with proper follow up

  • Weekly Medicaid Billing

  • Monthly Private Pay Billing and Collections

  • Work with attorneys for cases that need guardianship or private pay collections

  • Track new Medicaid applications in process, recertification’s and conversion from community to nursing home Medicaid

  • Participates in inter-disciplinary team meetings to discuss financial status and Medicaid eligibility

  • Demonstrates understanding and knowledge of Medicaid and Social Security Administration programs including eligibility requirements, application process, adheres to policies and mandates positive application outcomes

  • Documents all case activities in a timely fashion on appropriate forms

  • Communicates in a timely fashion with all effected departments to assure collection of accounts

  • Attends mandatory education programs as well as seminars, in-services and conferences when required

  • Associates Degree in Social Work or Social Services is preferred

  • 2+ years of Social Work experience in financial counseling and/or case management, preferably in a health care, Department of Social Services or a financial counseling setting

  • Knowledge of Medicare, Medicaid, Insurance and Social Security systems

  • Excellent interpersonal communication skills

  • Project management skills

  • Highly motivated self starter

  • Excellent written and verbal communication skills

  • Valid drivers license and the ability to travel within the surrounding vicinities

Hours of Operation

  • Our office hours are 8:00 a.m. to 5:00 p.m.

  • Our summer hours are 8:00 a.m. to 5:00 p.m. Monday through Thursday, and 8:00 a.m. to noon on Friday.

  • The ability to work additional hours during busy season is critical, but the rest of the year, flexibility is key.
