Planned Parenthood of Greater New York Patient Referral Coordinator in Queens, New York

Planned Parenthood of Greater New York is seeking a Patient Referral Coordinator to join our Clinical Services team for a full-time 37.5 hour work week. This position will be based at our Region 1 NYC & Nassau County Health Center.

In this role, the Patient Referral Coordinator is responsible for coordinating, processing and scheduling of medical referrals for patients in need of external consultation as requested by PPGNY medical providers. This position is also responsible for tracking and providing reports. The Referral Coordinator collaborates with social workers, nurses, and other patient care support staff as needed.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

  • Routinely contacts patients to remind them of their referral appointment by phone, mail, text, patient portal or other mutually agreeable method.

  • Answer, respond and document in the EHR phone calls, requests and questions from patients in a timely manner. Calls must be accurately managed or redirected as appropriate.

  • Assists patients in accessing other services needed to maintain their referral appointment such as but not limited to transportation, interpretation services, health care benefits.

  • Ensures that all referral forms and returning consultant reports are located in the patient's electronic health record in a timely manner.

  • Monitors electronic health record (EHR) notifications throughout the day to capture referral request in real time.

  • Gathers pertinent information as needed via EHR or from clinical team to accurately complete pre-authorization/referral process.

  • Schedule referral appointments directly on behalf of patient.

  • Utilizes third party payer/insurance portals; has an understanding of insurance providers, their portals and their expectations for authorization approvals.

  • Maintains an accurate records of external referral patient's appointments that are kept, failed and current status utilizing the EHR or other established system.

  • Responsible for running referral reports as requested.

  • Responds to in-house provider questions, requests and concerns regarding the status of patient referrals or follow-up status.

  • Accepts individual and joint responsibility for and participates in health care center efforts to comply with all organizational and quality assurance guidelines, and all applicable government regulations.

  • Meets Clinical Services Standards of Behavior.

  • Understands and commits to customer oriented approach to health care delivery.

  • Completes all responsibilities according to established protocols, policies and standard practices in the areas of ADA and regulatory compliance programs such as HIPAA.

  • REQUIRED QUALIFICATIONS

  • Minimum education

  • High school diploma/GED

  • Minimum work experience

  • Previous experience in a high-volume customer service role

  • Two (2) years of experience providing direct support to vulnerable, populations experiencing homelessness, substance use, mental health issues, and/or living with HIV/AIDS

TYPICAL PHYSICAL DEMANDS

Requires prolonged sitting and repetitive tasks including use of a computer. Frequent

standing, walking, bending. Must be able to walk up and down stairs. Requires lifting or

moving of patients/supplies/equipment. Visual acuity sufficient to perform frequent work

on a computer screen and review printed reports and other materials. Requires the ability

to hear and to communicate orally with others. This role routinely uses standard office

equipment such as computers, phones, photocopiers, and filing cabinets, and medical

equipment and will require reaching, grasping, pushing and pulling.

TYPICAL WORKING CONDITIONS

This job operates in a professional office environment. Potential exposure to communicable

diseases and other conditions in a health center environment. May require travel to other

health centers, at times in inclement weather. Requires flexible schedule and during peak

activity periods work in excess of 7.5 hours per day and/or 37.5 hours per week.

Proof of immunization or immunity to certain communicable diseases (including influenza during the flu season) and testing for tuberculosis. These certifications are required by the NYC DOHMH Health Code, NYSDOH and OSHA.
