When you join PPGNY as full-time you will work with the AIDS Institute grant-funded position located within Region 2N. The area covered by this grant includes the following counties and the PPGNY sites that are located within these counties: Fulton, Montgomery, Saratoga, Schenectady, Schoharie, Warren and Washington.
The LGBTQI+ Navigator provides navigation services to LGBTQI+ individualsacrossthisregion. This includes meeting with clients, assessing their needs, making LGBTQI+ friendly referrals and providing follow-up as needed. The primary goal of this role is to improve health outcomes and quality of life for LGBTQI+ individuals and their families.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
Identify and connect individuals with relevant community resources, health care services, support networks, behavioral health services, legal assistance and other referrals as needed (60%)
Research and develop a listing of health and human services referrals that are competent and respectful to LGBTQI+ persons across upstate New York (10%)
Stay informed about local, regional and national LGBTQI+ related services and resources (10%)
Participate in outreach programs and special events as needed to promote LGBTQI+ inclusivity and to increase awareness of PPGNY services (10%)
Complete all documentation and reports as required and within expected timelines (8%)
Assist clients in accessing and utilizing insurance coverage (2%)

Understanding of concepts such as gender identity, gender expression, sexual orientation and biological sex and able to clearly explain them to others
Strong understanding of LGBTQI+ issues and a commitment to promoting inclusivity

REQUIRED QUALIFICATIONS
Minimum education
High school degree
Minimum work experience
- Two (2) years relevant experience providing navigation or related services

Proof of immunization or immunity to certain communicable diseases (including influenza during the flu season and Covid-19) and testing for tuberculosis. These certifications are required by the NYC DOHMH Health Code, NYSDOH and OSHA.
