Here at St. Peter's Health Partner's, we care for more people in more places.Eddy Heritage House Nursing and Rehabilitation Center is a 122-bed skilled nursing care center located in Troy, NY. Situated on top of upper Tibbits Avenue, the center overlooks the great Hudson Valley. Dedicated to enhancing the dignity, comfort and quality of life for seniors in a homelike environment. Eddy Heritage House provides Skilled Nursing Care, Rehabilitation Services, short-term, sub-acute rehabilitation services for individuals who do not require the intensive rehabilitation therapy provided at an acute rehabilitation hospital and respite services.Why Heritage House? With its ideal location on a hillside above Troy, Eddy Heritage House offers residents panoramic views of downtown Tory, the Troy riverfront and beyond. We welcome two types of residents: those who are participating in short-term rehabilitation, usually following surgery, and those who are here for long-term care and truly become part of our family. Everyone can enjoy pleasant lounges and sitting areas for socializing with other residents as well as with family and friends. Outside there are spacious courtyards that offer a place to go and enjoy the great outdoors in spring, summer and fall. Assists with treatments and procedures and carries out selected duties within the framework of the established nursing plan of careResponsibilities:In collaboration with the RN collects data about residents and determines priorities is resident care based upon identified needs and goals.Contributes to the plan of care through collaboration and effective communication with the RN and other members of the healthcare team.Under the direction of the RN and within the scope of practice, complies with established nursing policies, procedures and standards to provide safe, quality care to residents.Administers medications and monitors IV therapy as ordered and according to established policies and standards.Provides emotional support to residents and their familiesWhat you will need:Required:A current license to practice as a Licensed Practical Nurse (LPN) in the State of New YorkPreferred:1 year of experience or more as a LPN, but willing to help train the right candidate to further their skills1 year of experience or more working in a skilled nursing facilityAbility to maintain the highest level of customer service while completing day to day duties and responsibilitiesAbility to communicate effectively in a fast pace work environmentAbility to engage within a team settingPassion to learn, grow and develop in the nursing fieldPay Range:$24.00 - $30.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
